The Doctor Who website has announced that Catherine Tate will be the new assistant. Also, Martha will be on several episodes of Torchwood and will then return to Doctor Who.

The new assistant has not been decided on yet, or at least not announced to the public. Although the actor of Martha's brother has dropped a few hints.
However, Martha is going to return later in the next series.

I don't know who the new one will be but according to the bbc news site Martha will be back as well as the new assistant a few episodes in to the next series.

Martha is not leaving. She will be absent from the Christmas special, but back for the new series next year.
