a note is just fine; surgeons usually make millions, so they don't need anything. It is the gesture of appreciation that is meaningful, so a card is with a handwritten note is completely fine. Actually, if you wanted to do a little more - sending them lunch would be appreciated, as they seldom have time to have lunch. Find out from the office what is a fav of theirs. Find out, too, where they will be and have it delivered to them; it can be delivered to surgery (which may be in the office) - that will be much appreciated. (send enough for nurses, too, if you can. He may have a scrub nurse that works for him)

I think it is a good idea to show your appreciation. Make sure you don't spend an extraordinary amount of money - if your surgeon has any ethics, he knows that it is inappropriate to accept a very valuable gift from a patient. This goes for anyone in healthcare

You could send him flowers or a box of candy. I've done it and my doctor was very pleased. It's never inappropriate to show gratitude, (except to a judge in a court of law -- they're forbidden to accept gifts).

Write a short, 1-page "thank you" letter, and offer to talk with prospective patients who are considering similar procedures. He can use it as a testimonial to show other patients. This will help him boost his business, and he should be very grateful for that.
