The remaining Jewish tribes in the world are Levi, Yehudah (Judah), and Binyamin (Benjamin).

The Kohanim (priestly class) are descended from Levi. Since they have special obligations and roles, they mostly know who they are. Regular Levites also have special roles, if not as much as the Kohanim. So it's safe to say most Levites also know who they are.

Some families can trace their lineage back to King David, who was from Yehudah. (And others can trace their ancestors back to famous people who traced their ancestors back to David).

I also heard that some last names have to do with what tribe you're descended from, but I haven't been able to confirm it.

Not really. It's too hard to trace, nobody knows what tribe they are from anymore. They can just assume they are from a tribe.

Due to the Roman destruction of jerusalem in 70 AD, most...if not all...of those records are gone.
