Your doctor is the best place to start - he/she will possibly organise either a referral straight to an orthopaedic surgeon or get xrays done first, then based on those, look at a surgeon for you.

Have you researched hip resurfacing? That is an alternative procedure that may be possible for you. Depends on how much damage has been done to the femoral head and acetabulum (socket). It will also buy you some time before you go for a hip replacement, as generally they can only do 2-3 THR's, depending on how much bone is cut away the first time and other technical aspects.

I run a group for adults who had perthes called Adult Perthes Survivors that you might find useful at http://au.groups.yahoo.com/group/adultperthessurvivors/
I've also set up an information site that might answer some questions for you http://perthesnz.bravehost.com

Start with your family doctor, he knows of all the orthopedic doctors in the area that do this work and who's the best.
He may have your doctor take X-Rays or MRI's to start the procedure then he'll set up an appointment with you for a physical and let you know what they saw, what the procedure will be, etc.
