Open Firefox and type in 'about:config'(without quotes) in the address bar. Now in the filter bar which comes up , type in 'browser.tabs.opentab' (without quotes). This will show up a preference browser.tabs.opentabfor. middleclick . This preference should be set to true . True is the default behaviour for this preference. This dictates that when you middle click a link it should open in a tab. If this preference is false, then when you middle click the links, they'll open in a new window instead of a new tab. So if this preference is set to false, just double click it or right click -> Toggle to set this preference to true again. This should solve your problem!
Also click on Tools -> Options -> Tabs -> Make sure the radio button 'a new tab' is selected under 'new pages should open in'. This ensures that the links from external applications open with a new tab instead of a new firefox window.

In Firefox, click on the Tools TAB, then, the TABS tab, and check the last (5th) check box, which states: "When I open a link, switch to it immediately". Then, a simple intuitive left-click will do what you want, automatically, with no need to even try to program your mouse wheel. Today's mouse models don't need their own Device drivers, they 'just work' in Windows, unless they are highly specialized "Gamer's tools". TIP: "I know for a fact there is a middle click, because I used to used it all the time before I got Windows 7." Best guess, this was likely an option, based on bundled software (or a required Device Driver) that came with a non-standard brand of mouse. This was not uncommon in the early days of Windows 9x.

Tools > Options > Tabs
Select Open Links In A New Tab. That way all links will open in a new tab instead of cluttering up your desktop with numerous windows.

Mr. Brightside is right, but you should also check the box that says: When I open a link in a new Tab, switch to it immediately.
It saves a few clicks for me.

install "Tabmix plus" and "colorful tabs" from addons.mozilla.org and go crazy with all the settings until it does exactly what you want, e.g. should it display the new tab immediately or not, etc.
