It would be helpful. Yes, I believe you do need that-for the paperwork. I had to do that for my cat. Also, proof of rabies shot-current one. If the dog is mixed breed, put down what you know it is, or if you don't ask-they may want to have you just put mixed.

Proof of rabies, distemper and the normal puppy shots. It is a lot cheaper too if the dog is spayed/neutered.

no, all you need is proof of rabies vaccination. Animal control doesn't care what breed it is.
Proof of spay/neuter if applicable.
