A search of MLB rules turned up nothing, other than a player must wear a helmet when batting, it said nothing about base running.
1.16 (a) All players shall use some type of protective helmet while at bat.

Yes they do. Even when pinch runners are inserted into the game, they wear batting helmets.

Yeah they do. During an Angels-Dodgers game this year one of the dodgers pitchers attempted a pickoff throw and it hit the runner's helmet. You don't want a baseball player to get a concussion.
