If you think someone has been abused or harmed, you should mention just the facts and avoid opinion. I have learned the hard way. Time, place, and evidence. A third party or proof will be necessary. Otherwise don't bother. If you are acting for someone else, you should try to get them to testify, if they are able, otherwise it may not be worth pursuing. This may sound harsh, but it will save you a lot of time and grief later.

Why are you involving yourself? If the abused person doesn't feel the need to talk to your boss about it why should you. Who appointed you departmental conscience? If you are the one who is being abused tell your boss that you may be forced to resign because of the abuse and see what response you get. At the very worst you can always 'vote with your feet' and walk out.

straight forth honesty and keep it simple. show respect to your boss and to your company at all times. don't make accusations that you can't back up. that seems to me to be the best course of action to take. good luck
