I noticed an ad format that I really like, that is not available in my Adsense format setup.

It's on the Wisegeek.com website - it's the large block on the top left, next to the picture. It has bullet points, large titles, and a tiny "ads by Google" notice at the top. It's set up on most of their pages - here is one example:

Does anybody have any idea how they did this?

You can get alternate ad formats like these if you become an Adsense premier customer - but your website will need to get a minimum of 20 million visits per month to qualify.

The owner of the site changed the code of his Adsense -- which is a violation and can be reportable to Adsense if you are so inclined.

I advice you that you do not follow his/her example and risk your account for tampering with the code -- which is a no-no with Adsense.
