Is there any other way to answer people when they say, "gosh, you're really skinny" than, "no not really". That's what I always say, but only because I can't think of anything else to say.

You could say, "gosh, you almost have manners."

If the people mean it as a compliment, say, "Thank you."
If its an insult meant to be mean say, "Gosh, you're really fat."

What you're saying now sounds fine to me.

Just say - 'Gosh, you're really rude, and is it an accolade to have halitosis that can be smelt a mile away'??

I guess you could just say thank you and leave it at that.
