Well some people use it as a scapegoat to say anything they want that's on their mind that is an insult or a cynical truth.

Really it's proper use is to redirect the meaning of a word or phrase so it is NOT taking in the wrong definition or sense.

Example: One man to a father "Don't take this the wrong way, but your daughter looks absolutely stunning this evening"

Usually it's because they are about to give you constructive criticism. Almost everyone will mistake it for an insult even though it really isn't.
This is why people prepare you for the constructive criticism by saying "Don't take this the wrong way but...."
The intent in constructive, the delivery will sound insulting, your pride will reject it, and your conscious will digest it.

They are trying to make them selves feel better about saying something that might upset you.

Ie:

Honey dont take this the wrong way but those jeans make your butt look fat.

Honey dont take this the wrong way but your hair looks like crude

you get the picture. They dont want to come off as rude, and want to make them selves feel better about saying it.

Long and short, the end of it for us who keep developing new ways to ask... though... it could also mean that you also approached it the wrong way. Expectations! "Yes, no, maybe, not sure, I am confused, I am mentally distressed, stop!...

They feel that what they are about to say to you you are probably not going to like. They think by saying "don't take this the wrong way" will take some of the sting out of it.

It means that they're about to tell you something that might hurt your feelings, but they are truly only trying to be helpful. They feel that you might gain some insight by their telling you and therefore save you some greater grief on down the road. They don't want you to get mad at them for telling you something they think you need to know.

It means that the person doesn't want to hurt your feelings when they tell you what they are about to tell you because they want to be straight up with you and not beat around the bush....Basically soften the blow when admitting something truthfully.

It means they want you to excuse them for being about to say something extremely rude and offensive, OR, they know there are at least two ways to receive a phrase they are about to deliver on your ears.
