Lifestyle change. What sports do you want to do? What will help you train for those sports?
Are you interested in soccer? Go practice kicking and you'll need to sprint a lot. In soccer practice we run and run and run.
Wanting to do football? Get some old tires set up in your yard and run that drill. Again lots of sprints. Good cardio like jump roping. Lots of upper body exercises too. Try to slowly build up to being able to do 150 pushups a day, 50 at a time. If you can't do it now, keep working on it until you can.
Watch what you eat. Cut out most sodas--you are still growing. Drink more milk and water. Cut out most fattening sugary snacks--no twinkies, swiss cake rolls, fudge brownies. That has no value.
Never miss a meal. If you want to lose weight, you need to speed up your metabolism. That means eating roughly every 2-3 hours. Fill a snack sized ziplock bag with dried fruit, fresh fruit like blueberries, a granola bar, veggies--even though salad dressing isn't that great for you if thats what it takes for you to eat carrots and cucumbers then put that ranch dressing on it.
You are at the age to develop habits for life. Exercise is about doing what you enjoy doing. Ride your bike around the neighborhood. Instead of video games, go practice your footwork cutting back and forth quickly.
Throwing a tennis ball against a wall is a great way to train for a lot of sports. It builds footwork, hand eye coordination and reflex time. Work on some upper body strength by lifting above your head. Don't forget to work on flexibility as well though. Stretch really well before and after each workout. It's the summer--swim some laps at the pool. Thats great for upper body strength.

fast food is salty food if you cut back on the salt in a few weeks youll be able to better taste the natural salts in food and may not crave the junk as much as you used to
