We're fighting an unconvention war and we're still learning how to combat terrorism--the "hearts and minds" approach. It's difficult to have a measurable ammount of "success" (failure or success is hard to define in terms of the battlefield) when you're learning as you go.
I'd say one of the biggest things that could help the situation is more forces to provide security. To win hearts and minds we need to improve their overall situation (utilities and the like), which we've been slowly trying to do. Challenges are that when we provide security while a massive utility project is underway we also have to move on to something else once the project is complete. That presents new targets for the insurgency, both where said project and the personnel who cooperated with us are concerned.
Also, one should note that democracy takes a great deal of time, particularly in the middle east. The two closest examples are Syria and Turkey, but they'll probably never be western style democracies simply due to cultural differences. It took us several years to come up with our current Constitution, it's foolish to think that it could be completed in another country with a totally different culture inside of five years.

We are fighting a guerrilla war , with conventional tactics, it did not work in Vietnam it will not work in Iraq, Troops fighting house to house with a full field pack on their back, There never has been enough boots on the ground in Iraq, to protect the targets of opportunity, that the guerrillas will.hit. A few bad apples in the military have given a bad image of our Troops. The Media needs to use common sense in its coverage of the war, they report that our troop favor torture and few days later 3 of our troops where capture, they paid for the presses folly,

...The usual... -An American military Convoy was advancing the current "Surge"- when it rolled over a big "Mine" in the road- and 5 of our guys were killed in the explosion. The "surge" of American deaths- continues. :(
