Right Start - they have an outlet cover switchplate that slides over and covers the whole outlet - when you pull the plug it slides over. They're great, and in my opinion much safer than the little stick in things.

Yes, or if it is an unused outlet you can get a cover that screws over it.

Also there are great covers for outlets you actually have things plugged into to prevent the plugs from being pulled out.

yes you can buy covers that completely cover the outlets. At your local hardware store or supercenter.
