Sometimes yes, especially if one has experienced it before, but many times someone who's fully insane doesn't realize it. It seems like the world has gone insane, and that gets one's attention so much that one may not seriously consider that it's not the world, but one's perception and cognition of it that's gone wrong.

well some people might say that if you are aware there's something wrong, that's a sign of sanity

on the other hand, i suppose that insane people can have moments of lucidity

so just ask a psychiatrist if you're concerned, better safe than sorry right?

Yes, in some types of psychosis, the progress is sufficiently slow and the person affected can feel the changes. I read about such but off-hand can't name them.
