Some pills should be taken on an empty stomach. Read the instructions.

Certain medicines need an empty stomach. Tetracycline reacts with milk products and becomes inert, for example. Other medicines upset the stomach if you do not eat something with them. Medications vary in absorption rates depending on whether you have food in your stomach or not, and some cannot be absorbed if no food is present.

If directions for medicine are food specific, it is a good idea to follow them.

Some medicine doesn't matter..Some of it,if you take it on an empty stomach, will make you nauseus or even give you cramps. It is just for your own ease.

I don't know if it's true for all of them, but some, like painkillers, can eat away at the lining of you stomach and that's why they are supposed to be taken with food or water.
