Doctors are trained to keep their emotions out of their work. Be straight to the point. Tell him and then say "I'll leave you alone with your family for a few minutes" and walk out. Go get a latte and return 45 minutes later. When he asks questions be vague and tell him you need to consult with another specialist but he's out of town right now and won't return for 48 hours. Tell him you have to see another patient right now and to please have his spouse call your office tomorrow to discuss payment options.

Don't do it. Instead ask your Doctor if he knows a good
psychiatrist then tell "Him "that your Doc has twenty four
hours to live. I'am sure He will think of a way to let him know.

Just divorce her; you don't need to kill her. Since she is a doctor, she can survive without you, unless of course, you are a multi-billionaire industrialists. In that case, I'm sure the courts will make an agreeable settlement. You can spare a few mils, can' you?
