To get cheese from milk, boil milk, add rennet (this used to be a piece of the stomach of a calf that had only had milk but now you can buy a chemical rennet). This will separate the milk into curds and whey. The curds are removed, salted, and then heaped into a wooden hoop that is lined with cheese cloth. The curds are then compressed for a period of time to remove as much whey as possible. Softer cheeses like cheddar are compressed for a shorter time than hard cheeses like parmesan. After the curds are compressed into a cheese "wheel", it is then aged and cured in a cool, dry environment.

Basically some sort of milk, salt, other flavors and/or spices. It goes through an aging process where lactic acid bacteria culture..thickening and flavoring the product until it ripens into a solid form. American cheese is different, it doesn't go through an aging process but instead is made of a combination of other cheeses.

It depends on what kind of cheese, i know cottage cheese is made out of sour milk to be honest. Then there is head cheese made out of you know what, and there are other things that cheese is made out of to but i don't know all of them.
