I received my new passport, but of course, there is no visa inside. My consulate told me I have to take action by myself and contact the USCIS. The problem is that neither the website nor the phone number gave me info as what I should do and I didn't succeed in making an appointment.
If only I knew where to go, I could wait on line until somebody could tell me what is needed and how to proceed to make a duplicata of my visa or to make a new one. But I don't even know where the office is. I really need help.

You can't get a visa if you are in the US, and you don't actually need on if you're there, since a visa is only for entry, not for stay. You should, however, replace the I-94 card, and you can do that using form number I-102:

...and you can replace the visa the next time you go home. You should report the loss of the visa to the State Department; the easiest way would be to email the consulate that issued it.

You will have to fax or mail a copy of the police report or affidavit of loss with the visa holder’s full name and date of birth to the Embassy’s Fraud Prevention Unit. One may also send them electronically via email address if the US embassy in your country has one.

Upon receipt of the notification, the visa will be rendered invalid for use to avoid its unauthorized use. In case the visa is recovered, one is advised not to attempt to use it as it has been invalidated.

There is NO AUTOMATIC REPLACEMENT of lost visas. One has to reapply for a nonimmigrant visa in order to make future trips to the United States by complying with the Nonimmigrant Visa Application Procedures.

What country are you in? If you are in the US, you can go to the nearest branch of the USCIS and they will help you. If you are outside, then call or visit the closest US consulate or embassy. The address should be easy to find on the internet.

Make an affidavit of loss regarding your passport with a visa. Then apply for a new visa and present the said affidavit and other documents.
