What you see on TV is, or at least should be, a cricothyrotomy, not a tracheotomy, and it's a last-ditch procedure, with the victim usually already passed out. Given the option, one uses local anesthetic with epinephrine even so, since the epinephrine in the lidocaine (or whatever) limits the bloody mess. Of course, the anesthetic isn't available in the TV scenarios where they're using a pocketknife and ballpoint pen in a bar.

Indeed it is painful, the reason why when it is done in non-emergency cases, general anesthesia is usually used. In very urgent and emergent cases however, it may be done without any anesthetic as the situation calls for. Saving the life of the patient would be the utmost concern and on top of the priority during the crisis. The patient may have varied degrees of pain but such pain for life would be worth sacrificing.If immediate relieve of airway obstruction is needed, saving the life will be more important than considering the pain it would cause.

it is possible to do a tracheostomy under local anesthesia, but i ain't gonna lie to you- it'll still be uncomfortable. if the patient hasn't received any sedation and he's not moving, it's probably because he's so hypoxic that he's not conscious. of course, if it's a fictional tv show, then it's probaly because the scriptwriters didn't know any better.

If it is the only way to relieve airway obstruction, then pain is secondary to saving a life.
Of course it hurts!
We give general anesthesia for scheuled tracheostomies in the OR.

Umm, no having your neck cut open without sedatives is a lot like being massaged by sexy naked people only it hurts like someones cutting youre neck open and usually the person doing it isn't sexy or naked. But the other thing is, if someone does this to you, just ttry to think about naked people masaging you and youll be good in a few minutes, and if not, the last thing you thought about was pretty damn happy!

Yes, they hurt. But if person is near death and needs one, chances are they are in an altered mental states due to hypoxia or most likely unconscious.

good kwerstion... the answer is: yes it hurts. anytime someone slices your neck open with a knife, theres got to be some pain involved.

If it is done wrong (and it is easily done wrong) it can kill. I suspect that it is rather uncomfortable but than it is also an emergency procedure
