Liver damage is the possible effect of acetaminophen. The maximum amount of acetaminophen for adults is 1 gram (1000 mg) per dose and 4 grams (4000 mg) per day. Taking more acetaminophen could damage the liver.
Side-effects of dextromethorphan use can include body rash/itching, disassociation, nausea as well as other gastrointestinal disturbances, drowsiness, dizziness, excitation, vomiting, blurred vision, sweating, fever, hypertension, shallow respiration, diarrhea, urinary retention, and increases in heart rate, blood pressure, and body temperature. It produces hallucinogenic effect if taken in high dose.

The cough and sore throat medicine you have I believe still has another component apart fronm acetaminophen and dextromethorphan. Anyway, if you take it in correct dose and accordingly, you need not to worry much on its side effects. Drink plenty of fluids and be extra careful if you are driving because of its dizziness and blurred vision side effects.

The Dextromethorphan can cause a wide variety of symptoms. Dizziness, drowsiness, Euphoria, Constipation, Inhibition of mucociliary clearance (anti-tussive portion). It also can cause hypotension, hyperpyrexia, nausea, myoclonic leg jerks when taken with MAO Inhibitors.

In an over dose it can cause convulsions and respiratory depression.
