On the tee, the ball has not yet been put into play, and simply bumping the ball off the tee is not an intentional stroke, trying to put the ball into play.
If you did this in the fairway, bumped your ball as you were setting up to it, then it would be a stroke.

There is no penalty on the tee.
Rule 11-3: "If a ball, when not in play, falls off a tee or is knocked off a tee by the player in addressing it, it may be re-teed without penalty. "
The ball is "in play" when you have made a "stroke". So for instance, if you whiff at your first attempt to drive, this counts as a stroke, so the ball is "in play". If you knock it off the tee after the whiff, there is a penalty.

No penalty. If you are setting down the club and knock it off there is no intention to hit the ball and thus is an accidental strike.

No. You can re-tee it without penalty or counting a stroke. You can even knock it off with a practice swing and re-tee it no matter how far or where it goes.

according to the usga, it is a stroke. i find it cheap. but luckly it never happened to me, yet.

it depends who you are playing with... if you are playing with some friends, then they might forgive your "accident" stroke. otherwise, yes. that is your first stroke.
