A pillow is useful for maintaining proper alignment of your cervical spine with the rest of your vertebral column. You should maintain the best posture based on how you sleep.

If you sleep on your side, you would want the pillow there to lift your head to the proper position. But if you sleep on your back, you may not need a pillow at all.

When it comes to pain from posture, you wouldn't feel it for years of continued misalignment. So just because you don't feel it now, doesn't mean you won't feel (and regret) it later in life

Hard beds usually require a soft, not too high a surface beneath, it helps a better breathing during the night, although its not a must, and it doesn't make a difference, unless you feel uncomfortable yourself.I don't use a pillow atall. No specific pros and cons, make sure if you sleep without a mattress below, you have something soft under your head.

If your body is going to be in one position for an extended period of time it is better if the spine is straight. Your airway also needs to be straight and open to allow good airflow while breathing. It doesn't have to be a pillow, any object that elevates the head slightly would do the trick.

Cervical spine gets extended under gravity. Pillow keeps it in resting position.
