So are you supposed to list the person's position?

Like:

Mr. John Smith
Supervisor, McDonald's
(555)555-5555

Even if John Smith doesn't work at McDonald's anymore? (He was my supervisor)

add the dates he was manager, if you know, for example

Mr. John Smith
Supervisor, McDonalds, 2006-2007
(add the location address)
Phone number

Add the location of the store, so if reference checker needs to verif that Mr. Smith was in fact a manager, they can call to verify that as well.

As a courtesy, make sure you have Mr. Smith's permission to use him as a reference, and let him know that prospective employers may be calling.

You might want to ask him for a reference in writing that you can use for future applications.

i still would. the mcdonalds you work at should still have a record of his information for any verification purposes necessary
