The best way for it to turn out well is for the cover up piece to work with the existing design. For some reason, some artists just start working and what still shows, just still shows and they don't get it.
Be sure the artist shows you lots of pictures of HEALED cover up work with before and afters - you will be able to tell right away if it was a bad cover job.
If there are heavy dark areas, for instance, then only heavy darkness can adequately cover it. We have sent some people for a little laser in some really dense areas and it helped a lot!
For you, you know your body best and your style and taste in clothing to determine an area you can easily conceal. Rarely is a tattoo permanently affected by pregnancy unless stretch marks occur around the hip area.

Excellent product in Australia that I know of called Dermablend.
It is avaliable world wide, in the US as well, which I assume is where you're from? Not sure sorry.

It's a beeswax based coverup and if applied properly, with the Dermablend setting powder to set it, should stay on all day. They have a face and a body cover.

Honestly with cover up, you never really know how they will turn out, there is always a chance the old tattoo will show through, He really needs to go into a tattoo shop and talk to an artist about it.
As for tattoo spots that wont be affected by future pregnancies, you could go with your shoulder blade, ankle, umm back of the neck, thats kind of hard to say, all girls gain weight in differnt places when they get pregnant.
