Just about every major hotel will have one although the quality will greatly differ from place to place. Also, most places now either charge a daily fee to use the facilities or even worse add into the daily "resort fee" which they bang everyone for and can be upwards of $20 a day. Make sure to ask about it when making your reservations.

Most hotels that have spas also have fitness centers. I have not been to a major one on the strip that didn't have a fitness center. I did see a comment about the Excalibur - while I realize that the theme, the building and the casino area are really cheesy - the spa and fitness center are not. The fitness center is big and bright with natural sunlight and I liked the spa better than the Venetian's.

Most of the new ones for sure. We just stayed at the Mandaly Bay for the 1st time last weekend - it surpasses the ones I've been to: Planet Hollywood, Mirage, Hilton, Luxo, Excalibur (just walked thru, but EW!) & NY NY.
Mandalay was great with EVERYTHING + it had a killer fitness center overlooking the beach!! ;)
