i'll be making the affidavit myself for my father and just have it notarized.

What you are asking for makes no sense.

Your father is older than you. How can you have a 'birth affidavit' that you make notarized? Notaries attest to the legality of signatures on the document.

It sounds to me that you are trying to make a phony birth certificate so you can 'prove' you are someone you are not.
