m = (0 - 4) / (12 - 0) = - 4 / 12 = - 1 / 3
y - 4 = (-1/3).(x - 0)
y = (-1/3).x + 4

(x/12) + (y/4) = 1

or x + 3y = 12

Yes, all straight lines have an equation like y = ax+b
So we know the point (0,4) is on the line, so
4=0a+b
so b=4
so we now have y=ax+4
we know (12,0) is on the line, so
0=12a+4
so a=-4/12 = -1/3
So the equation can only possibly be
y = -1/3 x + 4
.

y= mx + b m=slope
b= y intercept
the slope is y-y, over x-x,
the y intercept we already have.
so you get -

y=(-1/3)x + 4

you already gave the y intercept
