Well your question was not very clear but I am assuming that the link is what you are looking for. If not then please post more information or ask a clearer question.

Open Word (I don't know what you know)
Insert a 20 cell/Column table.
This table will have borders set around all cells. Remove the borders.
Make sure that you have formatting marks turned on (Tools, Options, View tab, tick Tab Characters, Hidden Text, Optional Hyphens and Paragraph Marks)
Click on the table then click on the table dropdown menu and select Auto Format. Then select None.
Now set the border for the top line, the end lines and the middle lines.
On the line below the table type the numbers.
You will have to mess around here to get the number to line up with the table lines.
Now at the bottom of th word window you will see Auto shapes and an arrow.
Click on the Arrow and click where you would like the arrow to start and drag to where it is to end.
Repeat this for the other side.

Done.
