It did occur, During the Second Punic War (218 BC) Hannibal led a campaign against the Romans and took about 30+ elephants with him. Many died on the way, however their were enough left to help Hannibal defeat the Romans at the Battle of Trebia. When Hannibal met the Romans in the famous Battle of Cannae, all but a very few elephants were still alive. Roman casualties numbered about 50,000 in one day of battle. This was the worst defeat suffered by the Roman Army.
