I think when God is punishing people, God does what ever he thinks will be the most annoying for someone. A hernia for you, might be hemmoroids for someone else. People can say that God doesn't punish us anymore, but of course he/she does, otherwise how would we learn. Like say you stole 20 bucks, you better believe that God will punish you somehow. Maybe by making you lose 40 bucks in a couple of weeks, that you were going to use to go to some concert or something. I think God divys out the Karma.

God Know everything. One thing that God does not do is punish. He set up laws and by the violation of the laws, there are consequences for our behavior. We end up punishing ourselves by neglecting to fallow the laws of the Universe.
God is an all Merciful God, An all forgiving God. A God of Total unconditional Love. God does not get anything out of punishing his children. He does not have to when we punish ourselves. God wishes none to parish. God is a Just God in that he allows us to use out free will to make errors and allows us a way out.

If you believe that God sends punishments then you won't like this answer.
I believe (those two words are crucial) that God does not send either punishments or benefits. We were put on earth with the ability to think and improve our lives. If we make poor decisions or our imperfect bodies fail (they will) that isn't God's direct intervention with us, it is just what we and our bodies do. Those who take their active part of their life out of their hands and attribute that to God are cheating themselves. Our faith in God is for the eternal salvation of our souls, not a means by which we can dump all our responsibility on God.

This material body is already a punishment, old age, disease and death, are forced upon this body by the laws of material nature. You take birth in this material world, that means that you are subjected to suffer. Is like the prison house, the prison is there, but any free citizen who become criminal is put into jail to be corrected and punished, but the government does not send him punishment, he just make the prison.

God does not send problems to punish you. That is just not true. Bad things tend to happen due to poor choices. If you are a believer and walk in faith then possibly obstacles can occur to guide you in the right direction.

God does not send punishments. If anything, He allows natural things to occur and hopes that we will turn to him for the strength to deal with whatever we must face.
His time of punishments ended with the new convenant in Christ...or so he promised.
