Usually God speaks with a thought planted in your mind referred to as a wee small voice. The devil has perfected the same thing. How do you know when it is God? If what you hear is not Biblical then it isn't from God. God will never impress you with anything that goes against His Holy Word.

I have a very serious answer. I know when God is talking to me from experience. I have heard God all my life, ever since I can remember. There is a still, small voice I hear in my heart, not through my ears. It is gentle, loving, and always correct. He sooths, calms, clarifies, and loves. Spiritual experiences are not easily lent to words. How to express feelings, unless you have felt them? Just because others have not "heard" this voice, does not mean it doesn't happen. It simply means they have not experienced it. If you can look back at some of my answers, I have left other comments on this subject. Hope you get what you need.
Blessed Be

God has spoken to me a few times.
I knew it was him, no doubt.
Once I was very depressed, kid just diagnosed with a life long disability, father had just died, my mother was not well and needed help, I was so tired I could barely get out of bed. I hated my life. I wanted to die. I would never kill myself but I could find no relief. I prayed in the car for God to take my life. I couldn't stop crying. I guess I was having a nervous breakdown but I really didn't have time for one. Then all of a sudden I heard a voice, in my head, loud and clear say, "But I have great plans for you!"
I almost wrecked the car!
Another time I was on the way to lunch with a co-worker and I heard that voice again say, in my head, "Talk to her about me". I thought, I'm just not in the mood, I don't want to. Then SHE brought it up! So I did.
For me it was really a voice that was in my head, quiet, but it was HIM!
I've had other experienced like this, write me and I'll tell you more if you want to hear.

God speaks to our hearts, you know it's God because He will never tells anything to harm us.
He does so when He wants to let us know what He wants us to do or how He feels about situations in our lives.
Just recently God put in my heart to teach His Word in Church and that's something I thought I would never do.
Satan will try to use your thoughts against you, such as discouragement, jealousy, depression., etc. that's not from God.
I'm wondering why more Christians didn't answer your question.
