You don't have to say that their baby is cute. Here are some alternatives for you:

Oh, he/she looks just like you. (be careful of adoptees)
Oh, that looks like one happy baby.
Oh, look at all that hair. (assuming they have any)
Oh, she/he is so tiny.
Oh, he/she makes me want to actaully keep my next one.

Ok, the last one is for people with a sense of humor, but there is always something you can say.

No matter what the child looks like, I always tell the parents how precious their child is -- because cute or ugly, that child is precious to those parents. Not a lie!

It's polite to, but you can be equally polite and work around caling the child cute. Not all kids are lookers lol. Comment on what the child is wearing ("Aw what a darling little sweater!"). You can also comment on the child's demeanor ("What a sweetie/little angel."). If they flat out ask you, "isn't he/she a cutie?" just be polite and say yes.

You don't necesarrily have to say it is cute, but saying it is butt ugly would be a little rude. Maybe if you don't quite agree that it is cute, just say "Ahhh" and let it go. Or smile and don't say anything. Don't feel pressured to say that something is cute if it isn't, but there is no reason to be rude either.

My favorite line for a child who is less than cute is just "awwww" - the mothers love it, and you don't have to say the baby is cute.

No, you don't. However, you don't want to just stare at the picture and say nothing. If the kid truly is NOT cute (and some of them just aren't), you could at least say something like, "Oh, look how curly her hair is!" or "How sweet!"

No, you can say something else:
How old is he/she? When is his/her birthday? If they have nice hair or eyes or a big smile you could compliment on that. There are lots of things to say without saying anything about their cuteness or lack thereof.

not unless you really think so other wise. be like oh wow that's a great picture who took it? or say something about the outfit, the background. Or just be like oh, that reminds me of blah blah blah. if the kids ugly, you could say it looks just like you. they'll take it as a compliment and lol

No. If it's an older child, just introduce yourself. If it's a baby, just say "you must be so proud."
