Well, it depends on the type of diet you are on:
If you are on a low-fat diet, then you should look at both fat and calorie content.
If you are on a low-carbohydrate diet, then you aren't required to look at either, just regulate the number of carbohydrates that you eat.
If you are on a zone-type diet, then you have to look at the total number of calories and balance them between the three types of calories (fats, carbohydrates and protein).
If you eat an unrestricted calorie diet that is low-fat, then you will definitely not lose weight. Your body can convert the sugar/carbohydrates that you eat into fat. If you had to choose between only regulating one, you should regulate the total calorie intake.

Both, most people only look at the calories, but fail to look at the fat that is in it, and sugar. The sugar you take in turns into fat, which most people also don't take into consideration. Don't just focus on calories, you might now lose weight.

All of the above, and then some. You want to make sure you look at calories, fat, fiber, sodium, etc...that's why all of that stuff is required on the label. Foods that are low in calories, low in fat, low in sodium and high in fiber are good for you. All of that stuff needs to be taken into consideration when buying healhty foods.
