Unless you are fighting the laws of gravity by trying to confine your breasts into a dress they shouldn't be in that shouldn't happen. Wedding dresses should be fitted to be snug around the areas they need to be without being super tight. Good support garments and a dress that fits well should prevent that.

Well, for one, start by ensuring that your dress fits correctly. The only reason the straps would break is because the dress was too small. You should go for fittings...when you do, ensure that the seamstress/tailor adjusts these correctly, and you might also ask them to reinforce the stitching where the straps connect to the dress.
The other thing you'll need to do is buy a good bra for underneath. Even if the straps do break, then you'll be covered by the bra.

That is a funny question. But honestly, before she march down the aisle. Be sure to advise her chaperone to check and double check that her straps is well secured. And God forbids if the straps still broke, the best you can do is just walk proudly (with your chin held up high) toward her, cover her up and continue with the wedding. Be proud of her exposed breast before the feasting eyes of others because in the end, they will be the one envying you feasting on those breast in your honeymoon.

Wear a corset or a boustiee underneath and tape those puppies in there.
The top of the dress also shouldn't fall if you have it altered to fit properly.
I have also heard of it happening but it is usually b/c of a bad fit and no underwear.

Well, wear a bra & buy a good quality dress. A good dress doesn't have that happen. I've never heard of that happening before, I'm sure you'll be just fine.

The likelyhood of that happening is very slim if you are wearing a well fitted dress. I think you just have wedding jitters. It will all soon be over and everything will be fine, just hang in there.

Well it sepends on what type of straps you have. If you want to take precautions, have a tug test and have safety pins with you(They do make ones in white).
