She needs to see a vet today! These are all symptoms of a blockage or a proliferation of the bowl. The black tarry stool is a sign of bleeding and distress somewhere in her bowl.
She is a puppy. She probably got a hold of something she shouldn't have. Please take her to the vet and please make sure you 'puppy-proof' your home.

Black poop means there is blood in it from high in the Intestinal tract, take her to a vet ASAP!
