Something short and sweet, obviously. It has limited real estate. But really, shouldn't you be the one to come up with the tattoo that's going to be on your body forever?
Be prepared to have to go back for touch-ups. Many people don't hold ink well the first time around.

My friend's husband got one done. The good news, he said it wasn't that bad. The bad news, is that now the first and last letters have worn off. Just make sure it's something that you really want.
