The medical record is actually a legal document that belongs to the entity responsible for maintaining the record, most commonly the hospital or in case of UK, it belongs to the Department of Health. The medical record does not belong to any doctor, though the doctor is one of the legal persons who records data. Though the data contained within the medical record belongs to you, the medical record itself does not belong to you but you have the right to retrieve data that is needed for your healthcare particularly.
You may go to the entity where you know your records are kept and follow due processes (some request form probably to be filled and approved by certain authorities). You need to know your medical number to facilitate the process of locating your record.

If all you need is immunization info, ask your parents where they have been taking you to get your shots. If they have gotten them in different places. The high school you just graduated from should have a copy of them. That is something they have to keep on file. OR, you can find out what shots you need and go get them again......Hope this helps.

Contrary to popular belief your medical records are not yours. They belong to the author of the record. Typically the treating physician.

You need to contact their office and ask for copies. Most of the time there will be a charge and you will need to sign a release of information form.

you have to go to the place where you got the shots and request them. the administration office of your high school will also have a copy of your immunization record ( that will probably be the easiest way to get a copy)
