"Come si chiama ?" can have different meanings.
One is the formal question to know the name of a person you don't know yet and then it means "What's your name ?" .
It can be also a question referred to a third person as
Come si chiama (lui/lei) ? What is his/her name ??
Another is the question you're asking to somebody to know the name of something. For example :
Ho visto un oggetto sul tavolo / I've seen an object on the table. Come si chiama ? / What do you call it ?

"I love you" is in Italian "ti amo" or "ti voglio bene" or "sono innamorato di te" or "ti adoro"
