I think the system has certain filters now. They're not 100 percent accurate, but they aim at taking the really offensive stuff before it events gets there. Sometimes, totally innocent things don't get posted, though - I'm guessing it's some kind of bot going through questions before they actually appear.
(You'll notice there have been less extreme troll questions for some time).

I don't know. How do they do it? Maybe they have access to the Mind of God wherein all foreknowledge lies hidden. Last time I looked at creation He wasn't much in the business of filtering things.
