D. Insanity defense contends the defendant is NOT GUILTY because s/he could not form the requisite mental state (mens rea) to be found guilty of a crime.

None of the above. You are talking "diminished capacity" which is not a form of the insanity defense. In fact, it's not a defense at all, it's a direct refutation of an essential element of the crime (mens rea).
