Don't punish him. He is a puppy and that's all he can do right now. He is like a baby for the moment. Plus he is probably getting in his adult teeth and teething, so he may be miserable...
Don't use ammonia to clean up any messes, because it smells like urine to the dog. Use vinegar or the special pet stain cleaners, instead.
First, clean the crate with soap and water, then clean it again with vinegar to eliminate the smell, then rinse it well with water.
1. Watch him very carefully when you are home and take him outside after he drinks, eats, or sniffs around. Keep him outside for 5 minutes, then come back in.... but keep careful watch. Always take him to the exact same spot outside for her toilet, and clean up solid waste every few days.
2. When you can't watch him, put him in a crate. You want to try to let him out about every four hours, but he is old enough that he can handle 8 hours at night, if you are diligent in walking him before and after meals, and before and after leaving for work/school.... and just before your bedtime.
3. However, he may have trouble holding it more than 6 hours during the day... You may need a little help.
4. If the crate is large, then block off part of it with a wooden board or sheet of cardboard. You can put bricks behind the cardboard to help hold it in place.
The crate should be large enuff to turn around and large enuff to lie down to sleep... but no larger.... kind of like a large bed.

The secret to house training your puppy to keep an eye on him all the time. If you know he goes to his crate to eliminate, catch him BEFORE he does so. Punishing him after he's already had an accident will NOT help him learn what you want him to do. First of all, your puppy should be taken outside (or to his appropriate "potty" area) at least every 30 - 45 minutes automatically. This is a sure-fire way to help prevent accidents where you don't want them. If you see him sniffing around acting like he needs to do his business - and heading for his crate - call his name immediately, and say something like, "Do you have to go outside?" Use the same phrase every time and eventually he will learn what you mean. When he responds to you after you say this, put his leash on and take him to his designated "potty" area. Praise him when he's finished - a young puppy will learn better if there's a small treat given as a reward. In time, your puppy will learn to alert you when nature calls. Almost no puppy goes through house training accident-free, so keep in mind that your puppy wants to please you more than anything. When he does, make sure you let him know. Punishment doesn't work nearly as well as positive reinforcement. Good luck!!

Ok this worked for me in about a week maybe 2. Ok when you take him out of the create you have to take him right out. When hes out of the create, i put him in a small room or where ever i am i keep a close eye on him. If i catch him in the act i clap my hand, and say no in a firm voice. pick him up and outside he goes. leave him out there,let him cry for 3-5 minutes, go outside and lead him to a spot you like him to go.NO KIND OF ATTENTION. untill he goes to the potty. when he goes praise him, you could even give him a treat. Also try to use the same door to go out! I also take my puppy out when hes done each activity,(eatting,sleeping, playing)
Does the create have a seporater? give him enough room to turn around comfortably and lay down. They will not go where they have to lay
Now if you leave, and he goes in his create do not punish him he may have done that a while back, they only remember back 30 seconds. You can only punish him for when you catch him in the act!

Try taking him outside into the yard every half hour and when he does his business praise him and give him a treat. if he does an accident in the house as soon as he has done it pick him up and put him outside without giving him any attention. You have to be really consistent but it worked for me with my puppy. They soon realize that they get attention for doing it outside rather than inside.

First of all get a kennel for him to sleep in at night just be sure its big enough for him to stand up and turn around in if you get it too big he will go at one end and sleep at the other defeating the purpose,then in the morning of course take him out of the kennel and take him outside then take him out again after he eats and every couple hours I also reccommed using the kennel when you aren't able to watch him or when you are at home. As long as he isn't constantly in the kennel and don't use it for punishment he shouldn't mind it. Just give him a week to get used to sleeping in it. and take him outside after he eats,and every couple hours while you are awake, consistency is the key

Actually sumtimes it can be seperation anxiety most times puppys want to be where you are so i recomend for a cuple of days find a blanket and lay it down beside your bed at night and let him go to sleep next to your bed. You mite see that he won't do his business on the floor when he knows you are there

Some people think it's gross, but my poodle puppy sleeps in the bed with me. Whenever he wakes up, I know it because I can feel him walking around and I take him outside.
Other methods I have heard of people using is: when they pee, rub their nose in it. They hate it and realize that you do too!

first how do you feed???
You should only feed him tice a day at teh most.
I only feed mine once in teh morning and teh food stays down and given only the amount the bag says for his weight.
mke sure he goes out just before he is pupt in the crate at night and first thing in teh morning.
Try this for about a week

when i night take away his water bowl and give him 2 to 3 pieces of ice...but don't hit him it wont work...and in the first thing in the morning take him out ..
