My Fuzzy was like that. He would throw the biggest fit when he saw the carrier. Oh! And then get him in the car? He'd howl at the top of his lungs. It was a horrible car ride.
Try this: take Jessi for short rides in the car. Don't put him in the carrier, per se. Just ride him around the block and then take him back inside. Work your way up and add a block at a time until you can take longer car trips.
All the while, try to break him into the carrier. Bring it out periodically and just place him in it with the door open. That shows him that he can get out and there will be less fear as far as that's concerned. Next, actually put him in and close the door. Pick up the carrier and just carry him around the house for a few minutes. Then let him out. Eventually, he will stop associating those things with fear.
Good luck!

When you say that you had to carry him into the vet, I assume you meant he wasn't in a carrier.

It isn't a good idea to take a cat into a vets without a carrier or at least a harness/leash. The cat could fight you and get away and you might not be able to catch him.

You also shouldn't put a cat in a car without a carrier or at least some sort of seatbelt type device designed for cats riding in cars. If you get in an accident, the cat could escape or be ejected from the car and you might not be able to catch him.

Sometimes if my cat fights me putting him into a carrier, I put him in it backwards. It's usually easier that way. Also, make sure the cat can't see out of the side car windows. Looking out the side windows can make a cat sick.

My cats hate the carriers too. You can put them in backwars if you would like or place the carrier up towards the ceiling and lower him in. Also for the car rides...my cat hates getting in the carrier but once in the car I will open the carrier door and keep it open and he has the choice of staying in or going out and he usually stays in but he just doesn't like to be confined. and I close the door before we get out of the car. As for the vet, my cat doesn't go unless he really has too because he attacks them and tears up the rooms so I give shots and all myself.

There are several things that you can do.

First ~ Do not use a sedative on your cat unless prescribed by your veterinarian.

Second ~ Start placing your cat's food in the crate. Make this the only place he can get his food.

Third ~ Leave the door open so he can go in and out whenever he wishes. Try to make it a place he can go to relax with a blanket and his toys.

He will soon associate his crate with other things than a car ride and the vet.

Go to Petco or Pet Smart and get a big cat muzzle. It covers both eyes.

Open the carrier from all sides (It takes two people to do it.)
Muzzle your cat and cover his eyes.
Wrap your cat in a towel.
Gently put him in the carrier and quickly put the lid on top and close the cage door.
As soon as he's in the cage, unmuzzle him.

The best of luck!

don't just associate the carrier with vet or car rides. just open the carrier and put its favorite bedding in there then let jessi just go in there on its own. if she has any favorite bed to lie down when jessie goes to sleep that would be easier instead of sedating or having to be rough with her. HOWEVER, you have to let jessi do it on her/his time.

He assoiates the carrier with the vet and he assoiates the vet with fear. There is not really a lot you can do for him short of getting a pet sedative from the vets before he goes down there. I would say it is something you have to put up with.

fear of the unknown most likely. Most cats dont like car rides. and if you have kept your cat from going upstairs in the past that may be why he is so shy or timid when in that part of the house.
