A lot of these issues were worked out in the steam era.
There are several different ways to look at efficiency.
Presumably you want to get maximum heat output,
compared to the amount and/or cost of fuel input.
First you have to analyze your alternatives & costs.
These break down to wood, coal, gas, oil, & electricity.
You'll have to plot their heat output against their cost.
A second factor would be the response time of the boiler.
Usually, this comes down to most surface area for volume.
This is accomplished by running multiple heating pipes.
This is so the heat produced by burning fuel is optimized,
i.e. gets fully utilized/absorbed by the water contents.
Third consideration, is the demand on the heated output,
e.g. what volume of steam or hot water is being drawn off.
The other side of that, is that cold water has to replace it.
In steam engines, this water was pre-heated & injected.
That utilized wasted steam & prevented cooling the boiler.

Improving the efficiency of boiler is becoming a wide spread concern.
I hope by 'boiler efficiency', you mean 'Thermal Efficiency' i.e. fuel-to-steam efficiency.
In order to IMPROVE the efficiency, first evaluate the existing efficiency of the boiler and then take steps to improve its efficiency.
The following are the key factors that play a vital role in the evaluatio of effficiency calculations:
1. flue gas temperature
2. fuel specification
3. excess air
4. ambient air temp
5. radiation and convection losses
Some simple industrial measures (among many many others) to improve boiler efficiency are as under:
1.minimize excess air levels
2.use feed water economizers for waste heat recovery.
3. proper timing of blowdowns
4. installation of boiler blowdown heat recovery.
5.minimize stack losses
6. treat feed water to avoid build up of scales and piting of heating surface
7. install condensate recovery system to pre-heat feed water
There are many other options available to improve the efficiency of a boiler.
In the end, I would just like to emphasize that Monitoring is the most important aspect of the task. Schedule a periodic monitoring plan to evaluate the efficiency

There are various aspects of boiler efficiency e.g
1.Thermal efficiency
improves by reduction of heat losses in radiation/ flue gas and ash , improved combustion efficency by better fuel, air/fuel ratio to limit excess air , operation at design load , waste heat utilisation by economiser/ aitheaters, optimised regenerative feed heating , Vaiable speed /frequency drive for large fans e.g I.D/ F.D /PA/ BFD pumps , condensate recovery and blowdown minimisation .
2. operational efficiency by
better reliability / availability , quick start up/ load throw capability by automation, plant capacity utilisation and high plant load factor, solid / gaseous effluent norm adherance capability , safety aspect and accident prevention .
3 . maintenace efficiency :
by minimisation of corrosion and tube failures by proper corrosion control and preservation .
preventive and periodic maitenance practices.
You can see that it is not only one aspect but a group that require to be taken care of to improve efficiency of boiler by a group . Each group need attension and improvement for overall improvement. Only joint effort of the O and M group / design group can fetch fruitful results .
