hi after a couple of **** ups i have had to review my asking out technique.

i reckon i need the following,

a rapport and interest

humour

and her name

these to me are all green lights have i missed anything ?

Clearly once past the logistics of status and name (with contact information), rapport is the key. You list humor and women love and appropriately funny guy where appropriate is defined by her.

Keep you expectations a little low and you'll be disappointed less often.
