in the ball/play area of a burger king, we were eating and heard liquid hitting the floor. There was a little girl pissing in the playplace.
when we told the people at the counter, they said that they'd wait for the next shift to come in to clean it because they'd already unclogged a stopped up toilet.
we tracked down the girls mother and she beat the poor thing.
all of it was just disgusting.

I saw flies and roaches crawling around! I got up and left. Gross! I've also seen a mom change her baby's diaper in booth. Yuck! Thank goodness I never saw a toilet plunger being carried around. I'm almost certain that's a health code violation.

It's always interesting when the server carries the drinks out with his thumb in them...
A girl with long hair that's not pulled back and it's dragging in the food that she's carrying on her tray at her shoulder.
A cook wiping the sweat of his forhead with his hand
A cook dripping sweat into the food.
A cook dropping food on the floor and picking it back up and putting it on the grill
A peice of hair inside of someone's hamburger
The nasty junk that comes off the soda and tea machines when they are cleaned.
A guy blowing his nose in a cloth napkin...this actually happened a lot.

A waiter with a bad head cold dripping Flem into my food while bringing my dish to the table. Bright red lipstick on the side of a so called clean coffee mug. A clean dish with cold mashed potato on the bottom of it in the buffet line!

Finding a looooong strand of black hair wrapped around a cigarette in my pork fried rice.
While standing in line at a buffet, I reached for a hot plate and found a deep fried cockroach on its back on the top plate.
As I stood there in horror, the cashier followed my eyes, reluctantly got up from behind the register and FLICKED IT off the plate. It gets better.
I went back to our table, emptyhanded.
My sister said, 'why are you so pale?'
I said, 'do you want to know now, or after you've finished eating?"
She said, 'tell me now'
I told her. She shrugged and continued eating.
She continues to eat there to this day.

Those two things you have listed are both as bad as I've ever seen- the only thing I would add is, why any owner of a restaurant would think it was ok to put actual guest tables, next to the kitchen, or worse yet, next to the restrooms.

a waitress picking her nose with her fingers, then washing her fingers at the water faucet! ( This was a buffet, the water faucet was among the fountain drinks, the ones that customers are able to get their own drinks) I almost threw up, told management, and have never gone back there, I told everybody that I know about that nasty waitress
