I am currently working for Lee Sandwich restaurant,the working is not safety enough for many people,it smell bad when you once step into the kitchen,trash are over the place,food had been storaged for a long time,in addition,they didn't provide enough equipment for employees,they didn't not wash the vegetable clean enough.......etc

The examples you give are more about food safety than employee safety. Report them to the local Health Dept. An inspector will come around and cite them for various infractions, in which they are ranked trivial, major, serious ... the result of this can be the restaurant has to close until some things are fixed, pay fines, then the inspector comes around a week later to see what's been fixed, then when everything fixed, the inspector only comes around again every few months or when someone calls the Health Dept again.

While I agree with the calling of OSHA you also need to contact the local branch of the Health Department about the mishandling of food.

Try OSHA, but make sure to make your complaint anonymously to avoid retaliation from your boss

talk to your human resources director, she/he will most likely direct you to the right person or help you themselves.
