Instead of stressing yourself out trying to figure out what she might like, give her a call and ASK her what she would like. Does she eat fish? Does she eat eggs and drink milk? Little questions like these can make a difference in what you serve and what she'll eat.
By asking her what she can eat, or what she would like to eat, you won't offend her, but rather, she'll appreciate you for asking, and for trying to accomodate her needs, as well as making her feel more comfortable at your BBQ.
IF she eats fish, I would grab some salmon, or whatever fish she likes, and grill that special for her, or add that to your BBQ menu for everyone to enjoy.
Portabella mushrooms are a favorite among lots of vegans, as well as non vegans. The mushroom caps are meaty and stand up well grilled. You can even use them like a burger, grill them, add some seasonings, then throw them on a bun with your favorite burger toppings.
Also think about side items. It wouldn't hurt to buy some squash and zuccini and serve them off the grill.
The macaroni salad sounds very good, but I would also offer a regular salad as well. Maybe your guests don't like macaroni salads, I know plenty of people who don't, but most people love regular salads too, and enjoy them with a great BBQ.

Grilled vegetables of course. Grilled apples are actually quite good...grill them in a foil packet for a great desert. Eggplant is good on the grill.
There are of course the fake meats...hot dogs, boca burgers (I reccomend Amy's brand though), boca even makes fake ribs. Fake meats will probably be the easiest since you can just throw them on the grill without much hassle. If your guest eats eggs, you have to make some deviled eggs (but please don't put ham or potted meat in them lol). Don't forget to have potato chips and bad-for-you desserts like brownies and cake!

You need to call and ask, I'm sure she'd appreciate the concern and happily give you some options. One thing to bear in mind, though, if your friend is a "heavy duty vegetarian" and has been for a long time... make sure to clean and scrape down your grill and keep a section just for her food and use seperate tongs. I have a couple of vegetarian friends who can't even eat anything that's been touched by meat proteins because their body can't process them anymore. You need to be careful so as to not make your friend sick.

Myself, I would buy a couple of veggie burgers, just for her. Or you could make her a grilled portabella mushroom burger. Veggie kabobs with tofu chunks is also a nice idea.
If she does eat fish, that is a wonderful option - a nice fish fillet or a shrimp kabob is great.
You want something that is filling...feeding her a salad isn't very satisfying!

I would recommend buying some veggie burgers and giving them to your family members to put on the grill. That way, you can still have an entree, but one that's meat-free. Also, you can opt for grilled vegetables. Try veggie kabobs. Sweet potatoes (and regular potatoes) are filling enough to be a meal, especially when paired with rice and other side dishes.

One of my favorites: potatoes on the grill!! The kids love these too...8x11 sheet of foil (double if using thin foil) Put a few slices of real butter on foil, add thinly sliced (unpeeled) potatoes and onions(peel those, of course!)...then a bit more butter; salt, pepper, perhaps garlic powder....I could certainly make a meal on this alone if I didn't want the meat!! Other additions: try green peppers or mushrooms!

Does she eat fish? If she does, grill some salmon fillets. How about tuna over tossed salad and some crackers. Make some Cottage Cheese Patties. (They are delicious) You can find the recipe for those on the internet, just enter it in your search engine.

veggies and humus....olives, french bread (no dairy)
if you have a nearby asian market, go find some pressed tofu...you can slice it and chuck it on the bbq and serve it on the bread with the humus.
any kind of fruit.....
does she eat fish? california rolls are good(sushi and cooked) find in the deli or take out area of your local store.
now if she is only vegetarian and not vegan your choices open up dramatically...if this is the case, dairy is in!

a big tossed green salad and a selection of fresh fruit would be good for all of your guests. Something like watermelon, cantaloupe, etc. I'll lay odds you have meat and macaroni salad left over, but no fruit.
