This could be any number of things. Torn meniscus in knees could cause swelling and pain, which you don't mention. You need an MRI, but I don't know if a clinic will do that due to the expense involved. Since it seems to be just joint pain, it is probably not your heart. It could be something as simple as arthritis. It certainly sounds like it needs to be resolved whatever it is, so ask them if they can refer you to a specialist.

You don't say your age or occupation. This all could have a bearing. I have the same problems. I also have osteo-arthritis in the severe degenerative stages. They don't know if these are related though. I do have "restless leg syndrome" there is no medical diagnosis for this. I take a pill called "Requip" for this, my sister has it also.
See your doctor, he would have to give you a deep scan, for blood clots or further tendon and cartilage damage. These are three dimensional scans of the soft tissues. If it shows no damage maybe you could ask for something like requip for a trial. They work for us.

Since you have problems in multiple joints, you need to think about a more generalized disease process, such as some sort of arthritis. Probably need to see a rheumatology specialist.
Good Luck
