The Church of Jesus Christ of Latter-day Saints has two distinct types of marriages.
"Sealings" or "Temple Marriages" have to be realized inside an LdS temple. This means that both the bride and groom must be LdS and be temple worthy. Any guests wishing to witness the ordinance must also by temple worthy LdS. There is generally a reception or dinner outside the temple for non-member friends and family. Sealings are valid for time and all eternity.
Civil Marriages are recognized by the Church. All Bishops are recognized as ministers by the USA and so they can perform the wedding ceremony. Bishops will not marry same sex couples, but other than that it's up to the government to set the rules. If the couple wishes to have a civil marriage within the temple, the above restrictions apply (current temple recommend for the couple and all guests). Civil marriages are only valid until death.

Nothing that would cause harm to the participants -- so, no animals or children (lack of informed consent and abuse of trust), and nothing between family members too closely related to each other (issues of intergenerational coercion and fitness to reproduce), among other things.
